Insurance:
Our physicians are Preferred Medical Doctors (PMD) under Blue Cross/Blue Shield of Alabama, as well as Medicare providers. For other HMO’s and PPO’s, please consult your insurance company to verify that our physicians are providers in your network. Please understand that the patient is still responsible for any co-pays, deductibles, co-insurance options, and non-covered appliances (slings, Ace bandages, post-op shoes, etc.), which should be paid for upon departure from the office.
If your insurance carrier requires pre-certification for admission or tests, it is the patient’s responsibility to ensure that proper pre-certification is obtained. If your claim is denied in part or in full, you will be required to pay the remainder.
Payments:
Payment for services rendered is the direct obligation of the patient. If you anticipate a large bill and you feel the need to work out a payment schedule, please talk with our Patient Representative, who will try to establish a method of payment which will accommodate your needs.
If you are injured on the job, please provide us with the name and address of your employer and/or Worker’s Compensation carrier. Should it be determined that medical treatment is not covered by Workmen’s Compensation, you will be responsible for payment of the bill.
Prescriptions:
Prescription refills, especially those for pain medications, will not be available during the hours that the office is closed, when patient records are not available to the physician on call. Please phone in your request during regular office hours. This policy is in keeping with the necessity of preventing medication errors and substance abuse.
